Below you will find a few links that will help expedite your check-in during your first visit to our office as a new patient. By clicking on the New Patient Information Form and Medical History Form links, you can print the forms and complete your patient profile prior to arriving at our office. This will give you plenty of time to provide accurate and complete information on your patient profile, which is essential for us to serve you well. You can also click on and review our Notice of Privacy Practices (NPP). After reviewing the NPP, you can then click on Acknowledgement Form link, print this form and complete this prior to arriving at our office as well. Again, this will just expedite your check-in during your first visit to our office. Also, you can click on the Financial Policies link, print them out, sign off on them and bring them with you to your appointment.
